We are welcoming you to join a friendly conversation with our Reach Out Oregon Staff.
Reach out Oregon Family Discussions are for families who have difficulties in navigating systems and are searching for educational and general support to barriers that exist in the mental health system.

Gentle and Restorative Yoga is designed for anyone, at any level or mobility. Gentle yoga can be done in a chair if needed and we have mats and everything you need. Yoga can be beneficial for pain relief, better sleep, increased immunity, strength, and all around well-being. Classes are $5 - $15 each session and meet on Tuesdays from 10am - 11am. Black Thistle Street Aid is an integrative team of volunteer medical practitioners, herbalists, & advocates providing access to free healthcare though direct outreach & pop-up clinics to people experiencing houselessness in the Eugene-Springfield area. Services we provide include on-site prescription refill assistance, wound care, foot baths, herbal and medical provider consultations, reproductive health needs, first aid supplies, and an array of harm reduction and hygiene/survival supplies.

Oregon has issued a new standing order that allows Oregonians to purchase contraceptives and birth control without a prescription (You can find more information on the Oregon Health Authority web page<https://www.oregon.gov/oha/ERD/Pages/Oregon-reduces-barriers-to-birth-control-for-Medicaid-members.aspx>). Most OHP members get their benefits through a local Coordinated Care Organization (CCO). OHP members can call their CCO and ask which pharmacy to go to. OHP members not in a CCO (known as OHP Open Card) or who do not know which CCO they're in can call OHP client services for help at 800-273-0557.

SOAP (Street Outreach Alliance Program) is a multifaceted, collaborative street outreach program. CORE partners with different agencies in the community to provide consistent and effective outreach to our neighbors living on the streets and in camps in the Eugene/Springfield area.
CORE serves people ages 16 - 29. Please reach out between 10am - 2pm on Tuesdays if you are in need of outreach supplies. You can text CORE at 541-870-0036, or visit their web page and fill out their service request form<https://docs.google.com/forms/d/e/1FAIpQLSdwiUA95ZXJKeY6CbdNIblvHdPaokADTRdzDCDUdoG-pg6U7g/viewform>.
Supply drops happen on Tuesday evenings from 4pm - 7pm if you are in need of outreach supplies.

Point of Pride's Electrolysis Support Fund provides financial assistance for permanent hair removal (electrolysis or laser) for trans folks who cannot otherwise afford care.
Join us for an info session! Our staff will walk through the application process and answer your questions:
* Saturday, January 24th starting at 1pm
* Tuesday, January 27th starting at 8am
You can sign up for the info session and apply for the program on the Point of Pride web page<https://www.pointofpride.org/electrolysis-support-fund>. Applications for the program itself are accepted from February 1st through February 28th. All applications must be submitted online.

Milkweed & Honey provides full-spectrum doula and peer services for young people ages 16- 29. Our team includes doulas and peer specialists with lived experience who walk alongside you throughout your reproductive health journey.
Services include:

* Abortion care and support
* Reproductive health navigation
* STD/STI Resource & Referral
* Healthcare/ODHS system navigation
* Support for young people ages 16-29 (housed, unhoused, and housing-insecure)

[HIV Alliance Logo] - HIVAlliance Behavioral Health Resource -
Our Behavioral Health program expands on our mission to not only support individuals living with HIV/AIDS, but to also provide culturally responsive services to the greater LGBTQIA+ communities. The program provides individual and family counseling services. We currently have openings for new clients 16 and older with Oregon Health Plan (OHP).

[cid:image046.png at 01DC8A23.960F3620] - Coordinated Care Organization (CCO) Changes for Oregon Health Plan Members Covered by PacificSource in Lane County -
PacificSource will no longer be a coordinated care organization (CCO) in Lane County in early 2026. OHP members in Lane County covered by PacificSource will have a new CCO: Trillium Community Health Plan (Trillium).
What this means:
Current PacificSource CCO members in Lane County will automatically move to Trillium and continue to receive OHP benefits.
PacificSource is not taking new OHP members in Lane County.
What you can do:
If you are enrolled with PacificSource in Lane County, you will receive a letter at least 30 days before you move to Trillium.
Please be sure to keep your contact information current so that you get this letter. Here's how: Learn how to send changes to OHP<https://www.oregon.gov/OHA/HSD/OHP/Pages/Report-Changes.aspx>.
The letter will say what to do to prepare. The move from PacificSource to Trillium will happen automatically.
This Does Not Change Your OHP Benefits:
Even if you change CCOs, your OHP benefits stay the same. You will not lose your health care.
PacificSource will serve current Lane County OHP members through the end of their contract. Please keep getting services as usual during this time. You can ask your health care providers if they accept Trillium. After you move to Trillium, Trillium will help you access the services you need.
When you move to a new CCO, you can still get care that was already approved. This is called a Transition of Care. It lets you keep getting those services for a short time, usually 30, 60 or 90 days, depending on your health needs and benefits.
If you have specific questions about pre-approved services, please call PacificSource at 800-431-4135.
About Current Health Care Providers:
Ask your current health care providers and pharmacy if they accept Trillium.
If they do, you don't need to do anything. You can keep seeing them.
If they don't, here are some steps you can take:
1. Ask if they plan to join Trillium's network soon.
2. If they won't take Trillium after you change CCOs, you might still be able to keep seeing them for a short time under OHP's Transition of Care rules.
3. During this time, Trillium can help you:
* Find in-network providers who meet your needs.
* Make sure your care continues without interruption.
Transition of Care:
If you change to a new CCO, you can still get care that was already approved. This is called a Transition of Care. It helps make sure your treatment continues without a break.
You can keep getting those services for a limited time. This is usually 30, 60 or 90 days, depending on your health needs and the type of care.
After Transition Of Care Period:
Once the transition of care period ends, whether you can keep seeing your current providers depends on if they are part of Trillium's network. To find out:
* Call Trillium at 877-600-5472 or
* Use Trillium's online provider search tool<https://www.trilliumohp.com/find-a-provider.html>.
Trillium is working to add more providers to their network, so it's a good idea to keep checking if your provider isn't listed yet.
Some Members May Be Able To Ask for and OHP Open Card:
This option is available at any time for members who have Medicare or are American Indian/Alaska Native.
In some cases, other members may qualify for temporary OHP Open Card if they have a serious health reason to keep seeing a provider who accepts OHP Open Card but not Trillium. You need to work with your provider to make the request.
