[Erate] Survey for FCC Broadband Comments - Urgent Action Requested!

The Oregon Department of Education will be compiling data to include in its response to the FCC Request for Comment on Broadband and e-Rate. There is precious little time to prepare these comments, but the Department is committed to including as much District level information as possible. The link below leads to a survey that will allow you to respond to any and all "questions" from the FCC that you would like to. This survey will compile your answers into a draft document that will allow Department staff to consolidate them and incorporate them into the response document. While there may not be space to include every single comment in the response, we will do our best to make sure that as many districts' ideas and scenarios are represented as possible. Because of the short timeframe and upcoming holiday, we need these responses no later than Friday November 13th.

The Department wants their response to represent the state as a whole, so your feedback is imperative to making that happen.

IMPORTANT - because of the way the form compiles the comments, it would be extremely helpful if you put your district name at the beginning of each comment box that you choose to enter data into. You may of course remain anonymous if you wish.
