I am saddened to report that there have been recent deaths related to blue fentanyl-laced pills in the Portland area, including students in our community.
These deaths are tragedies, and our heartfelt condolences go out to the families impacted. Sadly, substance abuse and overdoses are common in every community. These are unfortunate situations, regardless of the circumstances. Resources for Preventing Sleep Related Deaths
The state team would like to express gratitude to county teams for including prevention recommendations in child fatality review information submitted through the National Child Fatality Review Case Reporting System. By including this information in your reviews, you are taking an important step in preventing child deaths. In response, the state team would like to offer some resources related to these recommendations. Our first set of resources, below, is offered as a result of prevention recommendations from Clackamas and Union Counties regarding sleep related deaths. We know that Clackamas and Union are not alone in grappling with these types of deaths, so we are sharing resources here for all county teams who might want them.
1. Safe sleep self-study for Oregon’s family serving professionals. https://sharedsystems.dhsoha.state.or.us/DHSForms/Served/de2416d.pdf This safe sleep tool was created by the Child Welfare Division in partnership with family serving organizations and individuals across Oregon. This tool is a self-paced learning opportunity for professionals working with parents and caregivers and is focused on infant safe sleep practices, recognizing safe and high-risk sleep environments, and engaging families in conversations about reducing sleep related risk to the infants in their care. You are welcome to share with your team and your community partners to ensure families are receiving consistent messaging no matter who they talk to.

1. Safe Kids is an organization that supports safe sleep and may have resources. The Oregon Parenting Education Collaborative parenting educators have completed the safe sleep self-study and are also a resource for parent education – there are 16 hubs in Oregon that serve all 36 counties. For counties that have local public health, they are also an infant safe sleep resource.
2. There is a cross agency statewide safe sleep workgroup. The state team has requested the workgroup provide information to the county teams on what efforts they are working on. To inform their efforts, the state team has asked the workgroup to consider holding a listening session or find an alternate way to create an opportunity for county teams to share successes and barriers to safe sleep efforts.
Needs assessment update and request for input
In 2021 the State Child Fatality Review Team (state team) oversaw a contract with University of Oregon to complete a needs assessment of Oregon’s 36 county child fatality review teams (county teams).
Attached is a summary of the needs assessment results. In response to the needs identified in the needs assessment, also attached is a list of proposed resources and system improvements.
This is a request for county team members to review the proposed items and provide input. Do the items address identified needs? How would you prioritize the items? What is missing? Your voice is essential in this process.
Input is requested by March 11th, 2022.
