The National Center for Fatality Review and Prevention (National Center) is conducting an assessment to gain a deeper understanding of barriers that Child Death Review (CDR) and Fetal and Infant Mortality Review (FIMR) team members face in the review process when working to uncover factors that contribute to some groups experiencing worse outcomes than other groups. The National Center has partnered with the Michigan Public Health Institute Center for Healthy Communities (MPHI-CHC) to conduct this survey.  As part of the assessment, you are invited to complete this survey. Please use the link below to complete the survey. All responses are anonymous. The survey should take approximately 15 to complete. We appreciate you taking the time to share your important insights and experiences, your feedback is very valuable. Thank you!
