Job Details:


  *   Improves individual and community health through coordination of community-based health programs, such as Living Well with Chronic Conditions, and partnerships using the Community Health Improvement Partnership (CHIP) model as well as other public health program models.  Identifies, educates and engages community members (volunteers) as local leaders in improving community health. Cross-trains to increase team flexibility and be available to respond to community needs.

Bachelor’s degree in a Public Health related field required.
Experience and/or training in word processing and data management software required.
Living Well Master Trainer certification required within one (1) year of hire.

Master’s degree in a Public Health related field preferred. Experience and/or training in community-based health promotion program design, implementation and evaluation preferred. Bilingual preferred.  Experience and/or training in managing program budgets preferred. Experience managing grant-funded programs preferred.
  *   Weekend requirement - Occasional
