Position Title: CCO Community Health Partnership Coordinator
Department: Wellville
Title of Manager: CCO Community Health Partnership Manager
Supervises: Non-Supervisory
Employment Status: Regular – Exempt
General Statement of Duties
This position is responsible for helping oversee and coordinate activities
to assure that the goals and strategies of CPCCO’s sponsorship of Clatsop
County Way to Wellville are met. The individual performing this role will
serve as the local community representative of the CCO, to The Way to
Wellville Strategic Council, and other community stakeholders in Clatsop
County. This is a start-up position to help design, arrange or lead
activities that introduce community stakeholders to Wellville activities,
including supporting the work of the Strategic Council.
Essential Position Functions
§ Coordinate and help support Clatsop County community health initiatives
for The Way to Wellville
§ Support and assist with design, management and deliverables of the
Clatsop County The Way to Wellville Strategic Council
§ Serve as primary local contact for The Way to Wellville Navigator and
Wellville evaluation partner at the local level
§ Coordinate local evaluation components and assure alignment with HICCup
partners.
§ Coordinate grant and other funding proposals and submit, monitor and help
evaluate ROI and other funder-required measures
§ Oversee financial and other program reporting related to Wellville
funding/investments by external supporters
§ Help define improvement targets for the 5 Wellville outcome areas. This
includes helping identify and coordinate high impact local activities to
achieve those targets
§ Help identify and coordinate community stakeholders
§ Develop and maintain database of community individuals and partners who
participate in Way to Wellville
§ Be an official spokesperson for The Way to Wellville with the community
and participate in promoting and marketing Way to Wellville activities
§ Develop and help maintain relationships with the four other Wellville
communities
Essential Department and Organizational Functions
§ Propose and implement process improvements
§ Meet deadlines for completion of daily, weekly and month-end reporting
§ Maintain agreed upon work schedule
§ Demonstrate cooperation and teamwork
§ Provide cross-training on specific job responsibilities
§ Meet identified business goals that contribute to departmental goals
Knowledge, Skills and Abilities Required
§ Excellent listening and verbal communication skills
§ Excellent written composition/expression skills.
§ Refined interpersonal maturity/skills and a high level of
emotional/social intelligence
§ Understanding the nature of community engagement
§ Ability to work in an environment with diverse individuals and groups
§ Ability to build and maintain professional relationships with community
stakeholders
§ Skills in project management, group facilitation, community organizing
and systems-thinking
§ Ability to work independently absent explicit directions, excellent
judgment
§ Detail orientation and follow-through
§ Understanding Clatsop County and the potential pathways to health through
economic development, built environment, elected officials, social
services, health care, education and prevention/population health.
§ Requires irregular work hours and occasional travel (e.g., evening
meetings and weekend activities)
§ Ability to design Power Point and other presentation materials;
Familiarity with Microsoft Office applications, including Outlook, Word,
Excel, Visio and others
Education and/or Experience
Bachelor’s degree in Social Work, Public Health, Education, Communications
or related field required. Five years working experience providing the
ability to perform the essential duties of the position OR any equivalent
combination of experience and training that provides the required
knowledge, skills and abilities. Master’s Degree in a related field
preferred.
Working Conditions
The person in this position may sit for long periods of time while planning
strategies, attending meetings coordination meetings, answering phone calls
and questions, scheduling meetings and appointment and typing grant
applications, project updates, correspondence and other documents. She/he
interacts with a variety of internal and external stakeholders, which
requires a strong customer service orientation, good judgment, personal
initiative and a professional attitude and appearance. Changes are
frequent and time-sensitive, requiring flexibility and creative
problem-solving skills. The person in this position will be expected to
possess a high degree of initiative and motivation along with the ability
to effectively support and collaborate with others to achieve community
objectives. This individual will be in contact with a variety of
professional and community stakeholders and must be customer service
oriented and able to function independently and under potentially tight
time lines. Availability for evening meetings and for travel will be
required.
Equipment Used
PC and other standard office equipment for recording, storing and
presenting information.
