Dear Community Partners:

The Oregon Health Authority Office of Multicultural Health and Services, the Health Promotion Chronic Disease Prevention section, and the Northwest Health Foundation are working together to offer a funding opportunity to establish up to three Coalitions designed to address social barriers to health for underserved communities in Oregon.

We are holding Community Input Sessions for information sharing and feedback about this collaborative funding opportunity in your region in the second and third weeks of April. Input from these meetings, gathered from community partners and stakeholders will be considered for a Request for Grant Proposals (RFGP) at the end of this process. We will also host a webinar for those interested parties who cannot attend a meeting in person (more information on the webinar is coming soon).

To get more information and sign up to attend a Community Input Session, please follow this link: http://beattygroup.com/omhscommunityinput.htm. You'll be asked to register, then you can pick the session(s) you'd like to attend.

If none of these meetings times will work for you, pleas consider joining us for a webinar on April 20th from 2:00 to 4:00 p.m. If you would like to register for the webinar, please follow this link: https://www2.gotomeeting.com/register/462861882

Please also find attached background and rationale for the Coalition model. If you are able to attend one of these meetings, please review the two attached documents before the meeting. We will be answering questions about the background and rationale during these meetings.

Please note: Participation at this meeting is not required as a condition to apply for funding.
