We are pleased to announce that registration for the sixth annual Living Well Forum is now available online!  To learn more about the Living Well Forum, visit http://public.health.oregon.gov/DiseasesConditions/ChronicDisease/LivingWell/Pages/2011LivingWellForum.aspx.  You may also access the registration Web site directly at http://beattygroup.com/livingwell.htm.
 
Some of you may have already received an invitation from Christine Mebesius to register for the Forum.  If that is the case, please use the link provided in your email to register rather than the public link.  You do not need to register for the Forum twice.  If you did not receive an email from Christine, use the link above to register for the Forum.
 
As a reminder, hotel rooms will be covered for those individuals traveling 70 miles or more to Tigard, however, mileage and per diem reimbursement will not be provided this year due to budget constraints.  Also, if you are requesting a hotel room, you will be asked in your registration to reserve with a credit card number.  Although DHS/OHA will be covering hotel rooms for those individuals traveling 70 miles or more to Tigard, you will be charged for your room if you are a no-show for the Forum and do not cancel your reservation within the time stated in the hotel cancellation policy.
