Sounds like this may not have gone through on Wednesday so I¹m sending again
and extending the deadline to 11/28!

To TPEP and Healthy Communities grantees and tribes,

We'd like 5 minutes of your time to take a quick survey to select a new logo
for all the health promotion and chronic disease prevention work happening
across the state. This will be a resource for your use, so please take this
opportunity to state your preference!

If you have questions, please don't hesitate to contact your liaison or your
MG TA provider. We'll be finalizing the logo and building all the supporting
