[HealthyTribes] Survey for November TA call

Hello County and Tribal TPEP and Healthy Communities Coordinators,

Based on conversations with the Grantee Capacity Advisory Group, feedback from grantees, and HPCDP staff thoughts, we are testing out a new TA call format for the November TA call.
