Annual Tobacco Control Award Nominations
There is just over two weeks remaining before the deadline to submit
your nomination for the state wide tobacco control awards. Please take
time to think of someone who is especially deserving of any of the
awards below. This is a great time to reflect on all the amazing
accomplishments that have been made since our last statewide gathering
and honor those who have contributed so much to the health of our
community.
Please nominate recipients in any of the four categories (listed below).
Be sure to include a paragraph or two about why you believe your nominee
should receive the award along with the nominee's full name and contact
information. Awards will be presented at the Place Matters conference in
November. Please submit nominations with the attached form by Friday,
September 14th, 2012 via e-mail or mail to:

Maurine Neuberger Award for Distinguished Career Achievement - Maurine
