[HealthyTribes] CTG Small Communities - evaluation materials available on HPCDP Connection

Hi grantees,

We have posted additional evaluation resources on HPCDP Connection for use in planning Community Transformation Grant - Small Communities applications. The Performance Monitoring and Evaluation section of the proposal is worth 14% of the total points awarded to applicants. In addition to resources on HPCDP Connection, a representative from the our Surveillance Team will be available to review submitted drafts during the review period (July 16 - 20). If you have not done so already, please notify your liaison if you plan on submitting proposals to HPCDP for review.
