[HealthyTribes] Living Well funding opportunity

Dear Tribal TPEP grantees,

Attached is information about a small funding opportunity to expand availability of Stanford's Chronic Disease Self-Management Program (Living Well/Tomando Control) in Oregon.  Area Agencies on Aging (AAAs)<http://www.oregon.gov/dhs/spwpd/sua/docs/aaa-spd-offices.pdf> and Centers for Independent Living (CILs)<http://nwsds.org/content/Links/Oregon%20CIL%20Directory.pdf> are eligible to apply. Mini-grants of up to $5,000/year for 3 years are available based on funding Oregon Health Authority - Public Health Division received from the Administration on Community Living/Administration on Aging.  The deadline to apply is December 7. This is a great opportunity to work with your local AAA and/or CIL to help grow or establish programs.

The goal with these funds is to increase participation in Living Well programs by older adults and people with disabilities, and to increase the number of counties where Living Well workshops are offered at least twice a year.  As part of overall grant, Oregon's Office on Disability and Health will be helping provide training and assistance on outreach to people with disabilities.  While CILs are not required to provide matching funds, AAAs must commit to using their IIID funds to match requested funds.  CILs and AAAs that cover overlapping areas are encouraged to collaborate on efforts to make workshops available, and may both apply for funding.
