Good Afternoon,

Each year the Oregon Health Authority publishes Living Well Data reports using all the data we have collected over the year(s) from our licensed organizations state-wide.

The 2012 reports are now available online at our website: http://public.health.oregon.gov/DiseasesConditions/ChronicDisease/LivingWell/Pages/pubs.aspx.

As a note, the 2012 Living Well County Data Fact Sheets are available for only those counties that held at least one workshop in 2012 and submitted the data for entry into our system by January 11, 2013.  Any data that was received after that will be included in the following year's reports.  Other counties that offered a workshop in 2011 or earlier but not in 2012 continue to have access to their 2012 Living Well County Data Fact Sheets.
