[HealthyTribes] Title V Needs Assessment Survey

Hello HPCDP grantees,

Pardon any cross-postings.

HPCDP's partners in the Maternal and Child Health (MCH) Section of the Public Health Division are conducting a needs assessment survey as part of its work for the Federal Title V Maternal and Child Health Block Grant. The Block Grant, which funds a variety of MCH programs and services, requires that a needs assessment be conducted every five years. Results from these surveys, which are a part of Oregon's needs assessment, are essential for identifying statewide priorities for public health work on behalf of Oregon's mothers, infants, children and adolescents.

As state and local health promotion and chronic disease prevention efforts reach maternal and child health populations, we encourage you to participate in the needs assessment survey. The survey asks questions about a variety key maternal and child health topics, their importance to the health of your community/program, and the extent to which your community/programs currently addresses the topics. It takes 15-20 minutes to complete.

The online survey will be open for responses through August 21 at 5pm. Please contact Fred King (FREDRICK.W.KING at state.or.us<mailto:FREDRICK.W.KING at state.or.us>), if you have questions about the survey, the needs assessment or the Title V Maternal and Child Health Block Grant.

While some of the questions may not seems applicable to our work, I encourage you to answer what you can and get to the final questions at the end. In particular, consider the question below (which appears at the end of the online survey). Let me know if you have any questions.
