Hello HPCDP grantees,
Many of you are familiar with the Centers for Disease Control and Prevention (CDC) Worksite Health ScoreCard as one worksite wellness assessment tool. See below for information about a 2015 update to the ScoreCard.
The CDC Health ScoreCard is a tool designed to help employers assess whether they have implemented science-based health promotion and protection interventions in their worksites to prevent heart disease, stroke, and related health conditions such as hypertension, diabetes, and obesity.
In addition to the ScoreCard, there are three other tools that may be helpful in your work:
- Wellness at Work Website and Assessment

o Developed with Oregon stakeholders with a focus on on policy
o Assessment tool and website of resources
o Sections: Be Tobacco Free, Eat Well, Move More, Take Charge of your Health
- HERO Employee Health Management Scorecard

o Commonly used tool for large employers. Version 4.0 released in June 2014 to highlight organizational and culture support, program integration and outcomes.
- CDC Health and Safety Climate Survey (INPUTS)

o Assesses employee perceptions of work environment, working conditions, and the attitudes of your management and coworkers that support a healthy worksite culture.

[al?]The CDC Worksite Health ScoreCard (HSC) is a FREE online tool designed to help employers assess and improve health promotion programs in their worksites.
Enhanced Functions Added in 2015
The CDC Worksite Health ScoreCard Online system has added:
1) Additional reports that enable employers to look at specific worksite strategies in their ScoreCard(s) and benchmark within their own organizations and compare to other employers in the system.
2) Annual email reminders have been added so employers know when it’s time for their annual update and resubmission of the ScoreCard
3) Streamlined account set-up with optional demographic information
4) A detailed User Guide<http://www.cdc.gov/healthscorecard/usingscorecard.html> and a series of video tutorials<http://www.cdc.gov/healthscorecard/usingscorecard.html> that outline the features and functions of the CDC Worksite Health ScoreCard online system
Evaluating Health Promotion at Worksites
The HSC<http://www.cdc.gov/hsc> has been evaluated and determined to be a reliable tool for employers to use in assessing health promotion programs aimed at improving lifestyle risks and preventing chronic disease among employees. The HSC<http://www.cdc.gov/healthscorecard/index.html> helps employers identify gaps in their health promotion efforts and prioritize high-impact strategies for health promotion at their worksites.
This tool aids employers in assessing worksite health promotion by asking YES/NO questions across 16 health domains:
· organizational supports
· tobacco control
· nutrition
· physical activity
· weight management
· stress management
· depression
· high blood pressure
· high cholesterol
· diabetes
· signs and symptoms of heart attack and stroke
· emergency response to heart attack and stroke
· lactation support
· occupational health and safety
· vaccine-preventable diseases
· community resources
Spread the Word: Many People Will Find HSC Useful
A wide variety of professionals can use the HSC to assess worksite health promotion efforts, making the ScoreCard a great resource for CDC partners and grantees. Employers, human resource managers, health benefit managers, health education staff, occupational nurses, medical directors, wellness directors, and others responsible for worksite health promotion in an organization can use the HSC to establish benchmarks and track improvements over time. State health departments may assist employers and business coalitions in using the tool and identify ways of establishing healthier workplaces. State and local health departments also can use the tool for monitoring worksite practices, establishing best practice benchmarks, and to track improvements in worksite health promotion programs to more effectively direct resources to support employers.
