[HealthyTribes] Reminder of change in method for requesting data and evaluation support from HPCDP

Dear Colleagues,

HPCDP's new "Online Data and Evaluation Request System" goes live on August 1st.  As communicated in a previous email, this online system replaces the LETARF and SDARF forms.

The link to the online system will be posted on the Technical Assistance page on HPCDP Connection in August, or you can copy this address directly into your browser:   http://www.surveygizmo.com/s3/2360561/Data-request-form

Please note:

*         All requests need to be discussed with your HPCDP liaison before submission, to verify the work is within a HPCDP priority area

*         Allow at least three business days to complete a data request. If a request is urgent, talk with your liaison for assistance in determining a reasonable timeline

*         If the request involves creation and fielding of a survey, please allow at least three work weeks
