Dear Health System Transformation Partners:

Save the date for the Oregon Health Authority’s 2017 Innovation Café: Improving Key Health Metrics! For full details, see the attached save-the-date flier.

If you’d like to present a project that addresses one of the three topic areas below, please submit a brief proposal by March 10 at: surveymonkey.com/r/ICproject<http://www.surveymonkey.com/r/ICproject>

What: This is a forum for Oregon health system transformation champions to engage in peer-to-peer learning and networking. CCO, clinic and other innovative health system leaders will present projects and discuss learnings focused on three CCO incentive metrics:

·         Increasing effective contraceptive use to reduce unintended pregnancy

·         Reducing tobacco prevalence

·         Reducing emergency department use with a focus on behavioral health

Plenary speakers will include Ignatius Bau, a national expert who provides technical assistance to advance health equity; and Patty Cason, a consultant and clinician trainer who specializes in increasing access to comprehensive patient-centered contraceptive services.

When: May 9, 8 a.m.–4 p.m.
Where: Salem Convention Center (tentative)
Cost: Free
