Dear Tribal Partners,

The 2018 Oregon Place Matters Conference planning committee is seeking nominations for the Place Matters Awards.<http://beattygroup.cvent.com/events/place-matters-2018/custom-21-2a3c855eac554b70add83a3772e2b060.aspx> The awards will honor individuals, organizations, coalitions and businesses that have contributed to the health of our community through work related to chronic disease prevention, health promotion, early detection and self-management.

The planning committee is particularly interested in honoring efforts that advance policy, system, and environmental strategies to address the leading preventable causes of death in Oregon: tobacco, poor nutrition, physical inactivity and excessive alcohol use.

Award nominations are open for the following categories:


  1.  Joe Weller Guardian Award - This award will honor an individual who has demonstrated a commitment to mentorship of new advocates for prevention, early detection and self-management of chronic disease, particularly around risk factors related to excessive alcohol use, tobacco, nutrition and physical activity.


  1.  Outstanding Leadership Award - This award will be given to an individual who has demonstrated outstanding leadership in chronic disease prevention, early detection and self-management in the areas of leadership, building alliances, eliminating disparities and implementing creative, innovative solutions.


  1.  Outstanding Organization, Business, or Coalition Leadership Award - This award will be given to recognize organizations or coalitions that have implemented innovative, collaborative, multi-sector solutions to address chronic disease risk factors. This nominee should have demonstrated commitment by addressing barriers and implementing policy, system, and environmental strategies to reduce chronic disease disparities.


  1.  Maurine Neuberger Award for Distinguished Career Achievement - Maurine Neuberger was a pioneer in the Oregon tobacco control movement long before it was underway. Maurine pushed for stronger controls on cigarette advertising and co-authored the 1963 book "Smoke Screen: Tobacco and Public Welfare." In 1964 she sponsored and successfully fought for federal legislation requiring cigarette warning labels. This award will honor an individual for their long-term leadership, dedication, support and loyalty to the Oregon tobacco control movement. Nominees with more than five years of support will be considered.



  1.  Jane Moore Place Matters Award - This award will honor an individual or organization who has demonstrated long-term commitment, tenacity and leadership to ensure all people in Oregon live, work, play, learn and age in communities that support health and optimal quality of life. The award will honor an awardee whose work focuses on advancing policy, system, and environmental change to promote health and prevent and manage chronic disease. The awardee should be well-known and recognized for their passion, leadership, accomplishments and dedication to health promotion and chronic disease prevention.

Selection criteria:
An awards selection committee will include representatives from local public health authorities, Tribal health organizations, the Office of Equity and Inclusion and the Oregon Health Authority. This committee will establish selection criteria, review applications and recommend nominees to Oregon Health Authority-Public Health Division for final selection.

Submission deadline:
All nominations must be received by 5:00PM PST on Monday, August 20, 2018 to be considered. Nominations can be submitted by email, mail or fax.

Awards ceremony:
Awards will be presented in-person October 30, 2018 during the 2018 Oregon Place Matters Conference in Portland, at the Oregon Convention Center, 777 NE Martin Luther King Jr Blvd, Portland. Efforts will be made to ensure nominees are present to accept the awards.
