[LHDAccreditation] User friendly QI self-assessment tool?

Hi all, 
We have been trying pick a user friendly performance management
self-assessment tool.  It seems the following two suggested by PHAB are
not super user friendly and parts of each are useful.  
We feel going through the actual standards and measures has been in
itself a performance management self-assessment as we go along. 
Thoughts on this?  
Does anyone else have an example of a user friendly performance
management self-assessment tool other than the two mentioned below?
