The Quality Improvement plan is separate from the other 3 (CHA, CHIP,
Strategic) and not technically considered one of the 3 pre-requisites to
determine eligibility for accreditation.  
However, my understanding is that the CHA, CHIP and Strategic plan
should all be part of a health department*s formula to address quality
improvement, and developing these documents in addition to the QI plan
should be looked at as foundational to the accreditation process * and
therefore important to have in place before delving into the details. 
Measure 9.2.1 of the draft standards is *establish a quality
improvement plan based on organizational policies and direction*. It is
somewhat broad in scope and confirms a direct tie to the strategic plan
which focuses on agency priorities. I am inclined to develop the
strategic plan and QI plan simultaneously.

My understanding is there are 3 core elements required that have to be
in place in order to apply for accreditation - assessment, strategic
plan, and improvement plan.  

 

I have also heard talk about continuous quality improvement.  Is that a
4th area or embedded in one or more of the core 3?
