Jennifer is correct----that's what I get for reading my email too
quickly. The QI plan should be an integrated part of the Strategic Plan
but it is not one of the required elements for application. My
apologies.

The Quality Improvement plan is separate from the other 3 (CHA, CHIP,
Strategic) and not technically considered one of the 3 pre-requisites to
determine eligibility for accreditation.
However, my understanding is that the CHA, CHIP and Strategic plan
should all be part of a health department's formula to address quality
improvement, and developing these documents in addition to the QI plan
should be looked at as foundational to the accreditation process - and
therefore important to have in place before delving into the details.
Measure 9.2.1 of the draft standards is "establish a quality improvement
plan based on organizational policies and direction". It is somewhat
broad in scope and confirms a direct tie to the strategic plan which
focuses on agency priorities. I am inclined to develop the strategic
plan and QI plan simultaneously.

My understanding is there are 3 core elements required that have to be
in place in order to apply for accreditation - assessment, strategic
plan, and improvement plan.
I have also heard talk about continuous quality improvement. Is that a
4th area or embedded in one or more of the core 3?
