[LHDAccreditation] Utilizing the ACHIEVE Grant/CHANGE Assessment

We are using the same coalition group for our MAPP community assessment group.  The change tool did not capture the demographic informatin and looked at policy and environment mainly.  We are using the information collected, but need more.  Thanks

Subject: [LHDAccreditation] Utilizing the ACHIEVE Grant/CHANGE Assessment

I was wondering if anyone has been successful in incorporating the CHANGE assessments (part of the CDC's ACHIEVE grant) into their Community Health Assessment. I know there are a few other Oregon counties who have been ACHIEVE communities and I'm wondering if you've been successful in using that data as part of a broader Community Health Assessment. I know that the CHANGE assessments do not sufficiently meet PHAB's definition of a CHA but it seems like a lot of the information and partnerships could be coordinated into MAPP.
