[LHDAccreditation] Measure 12.3.2

Hello Listserv,
We have been studying Domain 12 and were wondering if anyone has recommendations regarding 12.3.2 - Track actions by the governing entity.  Documentation could  be meeting minutes, reports, dashboards, presentations, memos, or other record of discussion of governing entity actions.

Is anyone documenting this "tracking" and how are you doing it?
