[LHDAccreditation] Reminder: Accreditation Readiness Grant Application Deadline

Just a quick reminder that the Performance Management Program (PMP) is pleased to announce a Request for Applications for accreditation readiness funding. Detailed information and required application forms are available at: http://public.health.oregon.gov/ProviderPartnerResources/PublicHealthAccreditation/Pages/index.aspx

As an overview, PMP is planning to fund three projects, at a maximum of $10,000 each between January and June 2013. Funds can be used for developing accreditation prerequisites, implementing the strategic plan and/or community health improvement plan, quality improvement activities, or workforce development activities. Applications are due at 5 PM PST on November 30, 2012.
