*OPHA Performance Management & Quality Improvement Training Series*

**Apologies to those receiving this message twice.


As the last session in the OPHA Performance Management & Quality
Improvement Series, we are hosting a QI and Performance Management Round
Table discussion during the OPHA Conference on Tuesday October 9th
1:45-3:00pm.  *We are requesting volunteers to conduct informal 5 minute
"mini presentations" and to speak from their experiences and tie their
experiences back to one or more of the Domain 9 requirements.*

We hope for this to be an opportunity to share tools and templates, lessons
learned and to make connections with others working on QI and PM work so
that the sharing can continue beyond this training series.

Domain 9 Topics:

   - Engage leadership in establishing or updating Performance Management
   System
   - Engage staff in establishing or updating Performance Management System
   - Complete Self Assessment - Performance Management System
   - Assess customer satisfaction
   - Provide staff development opportunities on performance management
   - Have a QI Plan
   - Documentation of QI activities based on QI plan
   - Documentation of staff participation in QI activities (based on the QI
   plan)

group by *September 21st*.
