NACCHO is offering a discounted rate of $100 per participant. This webinar will explore the relationship between housing and health and ways to assess housing as a social determinant in your community, and will share quality affordable housing strategies that can lead to improvements in physical and mental health. Peer Learning Opportunities in Healthy Aging

In response to the interest in healthy aging, a second consultative and peer learning opportunity regarding Healthy Aging in community health assessment and community health improvement plans (CHA-CHIP) will be offered this year.  In a series of three calls, addressing the social determinants of healthy aging through the application of NACCHO’s Community Health Improvement Matrix (CHIM) to CHA-CHIP processes will be discussed. Calls will occur from 3:00-4:00 ET on Tuesdays: May 7, June 4, and July 2. Participants will gain experience in increasing community engagement and partnerships around the social determinants through use of the CHIM tool.
