Hi Shannan,

We do not use this form and I personally wouldn’t use it for NC’s program.  It reads very R&D focused which is not what we do in NC. I think the majority of my libraries would be confused by the form.

We use a grant agreement which has terms and conditions along with provisions in it with our libraries.  It is due for a review with our agency lawyers, but works well for us.
