[lstac] Debarment, Suspension, Ineligibility and Voluntary Exclusion form

We don’t use a separate form in Massachusetts but include the information in our contractual documents. When we award a subrecipient, we send them three documents: the state’s standard contract, LSTA grant agreement, and document about CIPA where they indicate their compliance status.

I’m attaching the LSTA Grant agreement. Page 1, 3, and 4 includes references to Debarment and Suspension. I took this form and adapted it from another state (North Caroline, I believe).

The state standard contract includes language around time duration, so we don’t make them sign it each year.
