[MyOEBB] Approval on Enrollment Request Screen

[MyOEBB system updates]

Good Morning,

 

OEBB has noticed that several educational entities are approving
requests in the Enrollment Requests screen in MyOEBB.  The action of
"Approved by Entity" should only be used if the person is being
grandfathered in with insurance and the educational entity has proof of
insurability from the carrier.  An entity should be able to provide this
proof upon request.  Failure to provide documentation could result in
denial of a claim.

 

If you have in error approved any requests on this screen please let
Cathy Cunningham <mailto:cathy.cunningham at state.or.us>  at OEBB know
immediately so we can undo this action.

 

The following amounts are Guarantee Issue:

 

Optional Employee Life (Active Employees Only) - $200,000

Optional Spouse/Partner Life (Active Employees Only) - $30,000

Employee Long Term Care - 6 years/$6,000 amount

 

There is not Guarantee Issue for Spouse/Partner Long Term Care.  Each
enrollment requires approval by Unum.
