[MyOEBB] Revised Open Enrollment Change Form

[MyOEBB system updates]

Dear Benefits Administrators -

We have revised the Open Enrollment Change Form to include space for members who enroll in a Summit or Synergy plan to select a medical home for each covered individual.

The same link on the Entity Forms page now points to the updated revision:
http://www.oregon.gov/oha/OEBB/entity/Pages/eeforms.aspx#Forms_for_Employing_Entities

We are working on making similar updates to the New Hire Enrollment Form and the Mid-Year Change Form. We will send another notification when those are available.

We apologize for our earlier oversight on this detail, particularly if you had already made copies of the older version for your employees. If you are not offering any Summit or Synergy plans in 2014-15, then the previous version of the form will still serve your needs.
