[OMS_MANAGERS] Community Survey

Hi Everyone,

I just jumped onboard with the City of Dayton.  With impending growth and development in our future, we want to make sure the community is comfortable with initiatives being pursued downtown.   Can any of you share a community survey your city may have created in the past that addresses the same.  I don't need the results, just the survey questions themselves.  Obviously some of the questions will be unique to your own city, but they too may spur thoughts of things we can add.
