If surname is not known, it is blanked.
OSF in title column
I know these ones aren't in instructions at all. I will take it up with
higherups.
Under no circumstances column B is to be used. That's house numbers.
Column 3 is Order of visits, and this is the one we want.
In surname list what's on the page. If it says St. Ores, list it as St Ores.
