It's that time of year again. 

Now that you've completed your 2009 year end reports I have one more for you to complete. A few of you have already sent the information. First, thank you for being so prompt. It's not necessary for you to report again. 

The 2009 numbers will be used for the 2011-2012 invoicing period. 

The 2010-2011 invoices will be issued May/June and will use the 2008 numbers you reported. 

Please take this opportunity to also update any of your contact information which may have changed.

You may submit your report by replying to this e-mail with the following information or filling in the attached form and e-mailing, faxing or mailing it.  

Note - your employer # for this purpose is your PERS # if you are a PERS employer - non-PERS employers have a four digit number assigned which begins with an "8".
