RE: Specific Learning Disability (SLD) Sample Form Updated

The SLD eligibility form has been updated to include additional and more specific references to the eligibility models currently being used in Oregon.  These changes were made to provide clarity around some of the more specific documentation requirements for each of the models. Specific major changes are highlighted on the attached version.

Reviews of these forms and the proposed changes were conducted at the November stakeholder's meeting.  Also, we appreciated the input and assistance of our Special Education Director Advisory.
