We are pleased to share information announced today by the U.S. Department of Health and Human Services (HHS) about the availability of approximately $32 million to improve the health of Americans through two Racial and Ethnic Approaches to Community Health (REACH) initiatives. These funding opportunity announcements (FOAs) are designed to support comprehensive programs that address health disparities in racial and ethnic groups. These two FOAs build upon previous REACH efforts and other community health programs that focus on engaging community partners to reduce health disparities.
REACH

*         Building on past accomplishments, the REACH FOA (approximately $20 million) will fund approximately six to ten organizations to implement sustainable practice- and evidence-based strategies impacting health disparities.  Applicants should be able to demonstrate national or multi-state reach through local partnerships or program activities. At least 75 percent of the funds awarded through this FOA will be disseminated to local partners. Awardees may choose to implement strategies that address disparities in cardiovascular disease, diabetes, breast and cervical cancer, infant mortality, asthma, or child and adult immunization.

REACH Demonstration Project

*         The REACH Demonstration Project (approximately $12 million) addresses obesity and hypertension prevention strategies, which are two of the leading risk factors for chronic diseases. Obesity and hypertension result in a high disease burden within racial and ethnic minority populations. This FOA will fund two to four awardees for a period of three years to develop and implement strategies that reduce obesity and hypertension in populations experiencing health disparities. Evaluation and dissemination activities will be emphasized.

Proposals for both REACH initiatives should focus on strategies that reduce or eliminate health disparities in one or more of the following racial and ethnic groups, including but not limited to: African American or Black, Hispanic or Latino, American Indian or Alaska Native, Asian American, and Native Hawaiian or Other Pacific Islander.
Both FOAs support the efforts of Million Hearts(tm), a national initiative to prevent one million heart attacks and strokes by 2016, by working to prevent heart disease and stroke within populations experiencing disproportionate burden of disease.

The letter of intent deadline for both FOAs is July 11, 2012 at 5:00 pm EDT and the application deadline for both FOAs is August 7, 2012, 11:59 pm EDT.
