Hello Community Partners,
I hope you all are staying safe and warm.
As we continue to navigate the challenges in our community as a result of our recent winter storm, we wanted to provide you all with some guidance to help people who are receiving SNAP that have lost their food due to having a power outage.
Here is some guidance that one of our managers put together.
Requests for SNAP replacement can be taken over the phone or in person. When over the phone, we can record the verbal attestation
General things to know about replacement benefits:
1. The request must occur within ten days of the food spoilage or loss
2. SNAP considers food spoiled after loss of refrigeration of 4 hours or more
3. Only the affidavit step needs to be completed to start the process. A list of lost food can be pended for.
4. Replacement benefits can be requested PRIOR to power being restored
* Example: Springfield resident has been without power since Sunday. They still have no power. They submit the request today, benefits can be reissued now, even without power being turned on.
5. Replacement benefits cannot exceed the SNAPS monthly allotment.
* Example: Jane receives $180 in SNAP monthly. She completes the replacement of benefits process and shows that she lost food valued at $400. Only $180 in replacement benefits will be issued.
* Example: Craig receives $200 on SNAP monthly. They complete the process and indicate that they lost food valued at $150. Only $150 will be issued.
Replacement Process
1. Oregonian completes a signed attestation or affidavit. The attestation can be on a piece of paper or can be on either the SNP-086 form in ONE by clicking on the “Generate Affidavit” button or the DHS 0349D<https://sharedsystems.dhsoha.state.or.us/DHSForms/Served/de0349d.pdf?CFGRIDKEY=ODHS%200349D,0349D,Affidavit%20of%20Loss%20of%20Food%20Due%20to%20a%20Disaster%20(SNAP)%20-%20NOTE:%20To%20be%20activated%20when%20an%20emergency%20is%20declared.,de0349D.pdf,,,,,,,,,,,../FORMS/-,,../FORMS/-,> available on the ODHS/OHA Publication and Forms website. A signature can be a wet signature or a verbal signature. (This is all that is needed to begin the process)
2. To complete the process the following items are needed. If they are not submitted with the affidavit, the request for replacement benefits will be pended.
* Verification of the disaster or misfortune. Staff processing the affidavit will attempt to verify the power outage online. This will be sufficient in most cases.
* A list of lost food that includes: Name of food, location of food at the time of loss and price of food

Hi Tricia
First I hope you and your family made it through the Ice Storm, we lost power a few times but nothing to complain about. I already have some Counselors at the Schools asking about replacement funds,
This is what they sent me
I saw something about extra TANF funds for families if they lost food due to power outage maybe Trista can help us figure out how to help them apply.
