Plaza de Nuestra Comunidad is seeking a full-time bilingual (English/Spanish) Social Services Case Worker to provide direct support and advocacy to Latinx and immigrant individuals and families facing social, emotional, economic, or health related challenges. This role involves assessing clients’ needs, developing service plans, coordinating community resources and monitoring progress to promote client stability, safety, and well-being. The case worker will primarily assist clients with OHP (Oregon Health Plan) and SNAP (Supplemental Nutrition Assistance Program) benefits. Through a team-based approach, they will also conduct Marketplace outreach and education to inform community members about available health coverage options and eligibility requirements.
