Good Afternoon,
In the spirit of collaboration (and trying not to duplicate efforts), Klamath County has allowed me to attach its newly created (and borrowed) HB2925 form for your use.
This is how Klamath County is utilizing the form:
Klamath County: Working with the jail and court administrator, Klamath County used Grant County's form as a model for its own.  The judges met yesterday and ok'd the form and  Klamath County is going to submit it with the charging instrument at arraignment.
