[Erate] SOEN Contract/SPIN Review Questions

Many of you who are using the SOEN contract have received review questions from a reviewer named Vibhuti Panchal. The questions are related to the old SPIN number that the Dept. of Admin Services stopped using years ago. I have communicated with DAS regarding this issue, and they are working to deactivate the SPIN number immediately in order to resolve this issue. With that in mind, the review questions should be answered as follows:

Is the referenced Form 470 is the establishing Form 470 for this service?

___ No, If no the referenced Form 470 is NOT the establishing Form 470 please provide the 15-digit Form 470 Number that did establish the bidding for this service. The establishing Form 470 is the specific Form 470, which was posted for that particular service for 28 days, and pursuant to which a contract was signed or an agreement was entered into.  The establishing Form 470 could have been posted by the State, if the requested services are being purchased off of a State Master Contract

X Yes, If yes, the referenced Form 470 IS the establishing Form 470, please proceed to the following questions.

A. Has the named contact person "Mark A Reyer and Patricia K. Middleburg" on FCC Form 470 (#120590000587293) ever been employed by or otherwise affiliated with Dept. of Adminstrative Srvcs. Information Resources Mngmt Di,  SPIN 143010176?

 X Yes or __No.

If Yes, please explain.

Please answer the following questions regarding the SPIN (Questions II to IV) and follow the corresponding instructions based on your answers:


II.   Please review your Form 470 contact information to determine the matches with the contact information for the service provider.  You can do this by using the by using USAC's SPIN Search Tool in the Reference Area on our website, http://www.sl.universalservice.org/Forms/spin_contact_search.asp to compare the service provider contact information to your own contact information.  If the service provider contact information listed on the SPIN Search Tool appears to reference you but it is not you and no other similarity exists, please explain.  A written statement is required.

If the service provider contact information listed on the SPIN Search Tool is incorrect (for example, the service provider has listed applicant contact information instead of their own contact information), contact the service provider and have them contact the USAC's Billing Collections and Disbursement Division at 888-641-8722 or the service provider can make the changes online at USAC's website at Changes and Corrections - Schools and Libraries - USAC<http://www.universalservice.org/sl/about/changes-corrections/default.aspx>.

A.   Is the SPIN contact information incorrect and will be updated? __ Yes X No.

If the service provider contact information is correct, but the SPIN is no longer being used because the company is defunct or out of business, please contact the USAC's Billing Collections and Disbursement Division at (see contact information below) or making the changes online at USAC's website at Changes and Corrections - Schools and Libraries - USAC<http://www.universalservice.org/sl/about/changes-corrections/default.aspx> to have the SPIN deactivated.

B.   Is the SPIN no longer active and will be deactivated?  X Yes __ No.

You may then ignore the remaining questions as they are not applicable in this situation. Send your response to the reviewer as soon as you can, and let me know if there are further issues.
