[HealthyTribes] Scheduling reporting interviews

Dear Tribal and County Coordinators,
It is time for the first in this grant year reporting interview on the TPEP grant. We are looking forward to hearing about the excellent work you have been doing and the challenges you face. We use the information gathered for tracking progress and also for making decisions on training content, resources, and state-wide strategy. Your voice contributes to our collective conversation.

Please contact your liaison to schedule a time to conduct your reporting interview. The report will cover the period of July 1, 2015 through September 30, 2015. The interviews should be conducted between September 15, 2015 and November 14, 2015. You can expect the interview to take approximately one hour.

Attached are the reporting tools for tribal and county TPEP that reflect the 2015-16 grant guidance. Please feel free to use the tool to make notes or gather documents prior to the call. Your liaison will complete the form during the interview and send the summary to you for your review and edits.
