Please help us spread the word to your health system partners about two upcoming pilot training opportunities focused on identifying undiagnosed hypertension.



The Background:

Did you know that there could be a difference of up to 13% between expected and observed prevalence of hypertension among your patient panel (Ciemins, Richey, Joshi, et Al., 2018)? Those undiagnosed patients may not be getting the care they need to manage their hypertension which puts them at greater risk of heart disease and stroke (Yoon, Fryar, and Carroll, 2015). Because of the known dangers, it is essential that we identify undiagnosed hypertension in order to connect patients to necessary treatment and supports early.



Through a grant from OHA Health Promotion and Chronic Disease Prevention, Comagine Health is piloting a training focused on preparing trainers to work with staff to develop policies and procedures for identifying undiagnosed hypertension in their communities. Through this training and CDC evidence-based materials, we hope more organizations will:
*         implement hypertension identification and control policies and procedures;
*         address accurate measuring and monitoring of blood pressure; as well as
*         make improvements in referral and connection to appropriate blood pressure control resources and services for: patients at risk of hypertension, patients newly diagnosed with hypertension and existing hypertensive patients.



Who?:

The intended audience for this training includes, but is not limited to, quality improvement staff, staff supervisors or trainers, medical assistant leads, community health worker supervisors, etc.



Although many of the concepts presented in this training refer to outpatient clinical settings, we believe this training could be adopted for other settings and staff.
