[LHDAccreditation] 1.2.4L question

1.2.4L#1 - Provide reports of primary and secondary data to the state
health department and Tribal health departments in the state.
Any suggestions on how other local health department's are showing
conformity to this measure?  
If the we/ the local health department are submitting our local data
for the state to develop the report, how would we demonstrate local data
submitted?  Would this be list/snapshot of data that gets entered into a
registry that helps to create the report?
