*Accreditation Announcements & Updates:

1. The Performance Management Program (PMP) announced a Request for
Applications for accreditation readiness funding. Detailed information and
required application forms are available at: *http://public.health.oregon.**
gov/ProviderPartnerResources/**PublicHealthAccreditation/**Pages/index.aspx*<http://public.health.oregon.gov/ProviderPartnerResources/PublicHealthAccreditation/Pages/index.aspx>
*.*  As an overview, PMP is planning to fund three projects, at a maximum
of $10,000 each between January and June 2013. Funds can be used for
developing accreditation prerequisites, implementing the strategic plan
and/or community health improvement plan, quality improvement activities,
or workforce development activities. Applications are due at 5 PM PST on
November 30, 2012.


2. Save the date for the next Accreditation Work Group Meeting on December
10th 1:30-3:30. Agenda and location information will be sent out soon. *Please
remember to respond to the poll on GovSpace or email me at
erin at oregonclho.org to vote on which two domains we should focus on at this
upcoming meeting. *


3. The new Accreditation GovSpace Website is becoming the place for all
things related to Accreditation and QI. This is the place to find
documentation examples and documentation requested from state
programs, templates
and helpful resources, and to participate in on-going discussions related
to accreditation and quality improvement. Please email Anona Gund at
anona.e.gund at state.or.us with your name, title, physical address, email
address and phone number to get signed up.
