[LHDAccreditation] Jackson and Josephine Counties CHIP Process

Hi all,

I'm going to be sending our CHIP partnership materials in a couple of e-mail because of the size of some of the attachments.

Please feel free to e-mail or call me if you have any other questions. I'm not afraid to share all that has been messy and difficult about this process.

Attached in this e-mail is the process proposal document that I put together after our December 2018 partner listening meeting. This then got shared with our Steering Committee and Jefferson Regional Health Alliance (JRHA) board in January.  I based some of the specifics of our approach on Maricopa County's approach in their first CHIP (I got connected with them through a MAPP training) as I felt that it fit with the varying levels of commitment and capacity around our partner table for engaging in collaborative projects.
