Hi Talela,

Attached is our amendment form.  It is traditionally used to amend grant awards for an increase in funds.  If other changes to the grant agreement are needed it can also be used. For instance, we had a library ask for an amendment regarding removing the words “required match” from the language in our standard agreement template for one of our ARPA programs.  We indicated that the required match was 0%, but they still wanted it removed.  Of course the agreement was fully signed and executed so we issued an amendment to change that line in the agreement.

Hi Talela,
I’ve attached a copy of our Grant Funding Change Letter Template. This is to be used only when the funding changes. I think you could modify it to accommodate other changes as well.  Funding changes are all I’ve had to do!
