[MyOEBB system updates]

Dear Business Managers, Superintendents, & Benefits Administrators:

 

It came to our attention recently that our Social Security Number (SSN)
consent form was outdated and did not match the current language in the
governing rule, nor OEBB's intent.  The updated SSN Consent Form is
posted on the Entity Forms page of our website at:
http://www.oregon.gov/OHA/OEBB/entity/eeForms.shtml.  We have also
attached a copy of the updated form for your convenience.

 

We ask that each educational entity read and sign this consent form
acknowledging the way in which OEBB requests and uses Social Security
Numbers.

Please complete the attached form (one per educational entity) and
return it by January 13, 2012, so we can update our files. You may mail,
