[OMS_MANAGERS] Light Pole Banner Program

On the city side, we have a Black History Month Banner Program and a
Veteran Banner Program. These are two things the DDA is considering taking
over, but the current City process would be one to copy if you like

   - Applicant provides information for the banner (photos, names, etc)
   - Applicant pays for the cost of the Banner ($100)
   - Public Works hangs the banners at the appropriate time
