Physical Restraint/Seclusion

In addition to submitting your data in Consolidated Collections, a few reminders:


  *   All agencies must submit and verify their submission from the 'Status Tracking' tab in Consolidated.
     *   Verify your submission by clicking on the small triangle 'bullet' located to the left of the collection name.

o   For June Exit and Child Find, check and verify each report.

     *   For Discipline and Physical Restraint/Seclusion, click on the verify submission button.
     *   You will know you have verified your submission if the submitter's name is listed in the name column next to the collection.
  *   Only June Special Education Exit will have a Final Submission Form, which must be signed, scanned and emailed or mailed to ODE by July 21, 2018
