[HealthyTribes] Interview reporting time

Hello all,

It is once again time for interview reporting for TPEP. Please contact your liaison to schedule your reporting time. As a reminder, we shifted the timeline for TPEP programs so that interview reports occur shortly before you are responding to your RFA. Please complete your reports by the end of January. For those counties and tribes with a Healthy Communities grant, those reporting interviews will be conducted between February 15 and March 15 for this year.

Attached are:

*         A chart of liaisons along with their contact information

*         The Tribal TPEP reporting form

*         The County TPEP reporting form
Both of the reporting forms are the blank versions. Because this is your second quarter, information will be added to the form you used previously rather than this blank form. It is included here for your information but please work with your liaison to use the form specific to your tribe our county.
